Under National Healthcare Group (NHG) DEFINITE (DiabEtic Foot IN prImary and TErtiary) Care, a multi-disciplinary primary and tertiary clinical workgroup, the Diabetic Foot Coordinator (DFC) with work closely with NHG Polyclinic Family Physicians, Vascular Surgeons, Podiatrists and fellow DFCs at Tan Tock Seng Hospital, Khoo Teck Puat Hospital and NHG Polyclinics to assist in delivering care for patients with diabetic foot conditions.
ROLES & RESPONSIBILITIES
The DFC is expected to coordinate care for patients with diabetic foot conditions in the polyclinics, hospitals and community and act as the liaison person between the patient and / or caregivers and their doctors, podiatrists and all members of the healthcare team. It is also expected of her / him to participate in the following :
- Perform assessment, documentation, monitor, intervention, review patient’s needs throughout the continuum of care and post-discharge.
- Provide education / counselling to patients and / or caregivers.
- Conceptualise development, implementation, evaluation of new strategies, procedures or systems in collaboration with healthcare team to improve clinical service and care.
- Support quality improvement and academic research by working with fellow DFCs, Research Assistants and Clinicians to design, plan, collect data, monitor and coordinate related clinical research projects / trials, consenting patients for enrolment into the studies and registry, and any other tasks as assigned
JOB REQUIREMENTS
Degree / Diploma in a science or health-related field.Minimum 2 years and 5 years of clinical / research experience in the healthcare sector for Degree and Diploma holder respectively.Not adverse to review and manage diabetic foot wounds.Team player, able to work independently and meticulously with good organisational and planning skills.Good communication and interpersonal skills.Proficient in Microsoft Office applications (Microsoft Word, Excel, PowerPoint) is essential.Familiarity with healthcare systems, research statistics programs such as SPSS or Graphpad Prism and research registry systems such as REDCap will be an advantage.
