Palliative Care Practitioner / Palliative Care Medical Officer

Penerangan pekerjaanLiaise with referral doctors on the patient’s medical condition so that proper care can be administered. Participate in the planning, implementation and activities related to promoting the growth of ASSISS Palliative Services (APS) and ADOPTs. Discuss with the Head of Department of APS with regards to patient care needs and should there be any arising problems. Provide palliative care to patients and their families by regular contact and visits to monitor the patient’s condition. Conduct bereavement visits to families following a patient’s death, and arrange further follow-up when necessary. B. Professional / Procedural Aspect Conduct thorough assessments of patients’ medical histories, physical conditions, and specific needs to develop and implement individualized care plans. Communicate with patients and their families to discuss treatment options, goals of care, and advance care planning. Provide emotional and psychological support to patients and their family. Assist patients in making informed decisions about end-of-life care options and support them in achieving their preferences. Collaborate as part of a multidisciplinary healthcare team, working closely with oncologists, nurses, social workers, chaplains, and other professionals to deliver comprehensive care. Utilise pharmacological and non-pharmacological pain management techniques to ensure optimal comfort and pain relief. Assist patients in creating advance directives and living wills, ensuring that their care preferences are documented and respected. Stay up to date with the latest developments in palliative care, contribute to research initiatives, as well as provide education and training to colleagues, students, and the wider healthcare community. Palliative Care Practitioner : Diploma in Nursing; Registered with Malaysia Nursing Board and possess valid APC Palliative Care Medical Officer : Medical Degree (MBBS / MD or equivalent) from a recognised institution; Registered with the Malaysia Medical Council (MMC) and possess valid APC Possess own transport Strong decision making with problem solving skills Strong interpersonal skills and people-oriented
To support people with life-limiting illness, manage their symptoms, and improve the quality of life through thorough relief of pain and other distressing symptoms. Comfort and care will also be provided.

Responsible for the management and delivery of palliative care in a community setting.
Conduct thorough financial assessments to identify and prioritise patients who are in need of free healthcare services.
